Arrhythmia, heart failure (hospitalisation for moderate to severe heart failure in the previous 12 months), myocardial infarction in the previous 6 months Monitor body-weight, bladder outflow obstruction, paradoxical bronchospasm, prostatic hyperplasia, susceptibility to angle-closure glaucoma
